PS4 and Logitech Formula Force EX: No Throttle or Brake Pedal

Ciao,
Firstly I want to say great work creating this tool!
Unfortunately I'm having some trouble getting my steering wheel setup to work.
Here's the details:
Setup:
As per instructions on: http://gimx.fr/wiki/index.php?title=DIY ... or_dummies
Teensy ++ 2.0 with correct file at90usb1286.hex loaded
RX pin D3 TX pin D2 (Correct, tested stops working if swapped!)
USB 2.0 to UART TTL 6PIN Module Serial Converter CP2102
Steering Wheel: Logitech Formula Force EX
Game System: PS4 (Original) currently with game GT Sport
Control: PC with Windows 7 Professional
Issue:
The steering wheel buttons are not mapped as expected and neither the brake or throttle pedal work.
- GIMX-Launcher Check is OK (Output = GIMX adapter, Port = COM3, Input = Physical devices, Config = LogitechDriverForceEX_G29.xml, GetMouse = Checked)
- Regarding Config files - For the Logitech Driving Force or Formula Force all the config files are the same except name (as you would expect)
- In GIMX-config Axis seem fine, Auto detect button for an axis then message in bottom bar disapears when activating axis
- All checks work and I connect to the PS4 Dual Shock and control the PS4 with the steering wheel d-PAD
- All steering wheel functions calibrate fine within the Logitech Software (Logitech Software reconises the wheel as 'Driving Force USB'
Steering axis is fine and feels good.

For reference the button naming in the Logitech software is as following:
Button Wheel
1 = Cross equivalent
2 = Square equivalent
3 = Circle equivalent
4 = Triangle equivalent
5 = Shift Up (Back Right)
6 = Shift Down (Back Left)
7 = As desired (Upper right)
8 = As desired (Upper left)
9 = As desired (Lower 4 LL)
10 = As desired (Lower 4 LR)
11 = As desired (Lower 4 UR)
12 = As desired (Lower 4 UL)
(13?) = Steering Wheel
(14?) = Accelerator
(15?) = Brake
D-Pad = POV 1 (8 way)
My fingers are crossed for a simple and quick fix!
